This is an Animation Cel (which is upside down) from episode 123 "Zach and the Alien Invaders". Wingnut (the character in this cel) is hanging from the ceiling just after the Turtles drive off and Zach is strapped in a chair. Wingnut peels off his skin and jumps up to the ceiling saying "Hey if it's alright, I'll just hang out for awhile".
